This is an upgrade fee to make your scheduled appointment a home visit. For those who cannot or prefer not to travel to Dr. Forrest's office, Dr. Forrest will visit you at home. [only available within a 30 minute radius of Sedona, AZ.] PLEASE NOTE YOU MUST ALSO SCHEDULE AND PAY FOR AN APPOINTMENT. This is the additional charge to make an appointment a home visit.
